Free Birth Certificates for Homeless Individuals

The New Jersey Department of Health, Office of Vital Statistics and Registry, is offering free certified birth certificates to homeless individuals and shelter residents born in New Jersey, if requested by an authorized agency representative.

All requests must be submitted on letterhead and signed by the social worker providing assistance to the individual or the program coordinator of the shelter where the individual is temporarily residing. Requestor must attach a copy of their photo work ID proving affiliation with the requesting agency. Requests cannot be made by the individual in need of the birth certificate.

The client must sign a delegation of authority form and is entitled to a one-time request.

The first copy will be furnished without fee. All additional copies will be subject to the statutory fees.

Emergency Shelter

The request must be transmitted on the emergency shelter’s letterhead and include the shelter’s employer identification number and an attestation by the coordinator that the person is currently homeless and residing at the shelter. Requestor must attach a copy of their photo work ID proving affiliation with the requesting agency.

Social Service Agency

The request must be submitted on the social worker’s agency or professional practice letterhead and include the agency’s or the professional practice’s employer identification number and an attestation by the social worker stating that they are working with a person who is currently homeless. Requestor must attach a copy of their photo work ID proving affiliation with the requesting agency.

In-Person Requests

In-Person requests can only be made at:

In person requests will take 2 hours or less to receive birth certificate. The homeless individual is not required to come in person when request is being submitted. A staff member can make delivery and pickup.
